Assumption of Risk

I understand that participation in activities offered by The Green Room Theater and Dance, including rehearsals, classes, camps, workshops, performances, movement activities, dance, games, use of props, costumes, backstage areas, and outdoor activities, involves inherent risks of injury. These risks may include falls, collisions, physical exertion, equipment related injuries, and other unforeseen accidents.

I voluntarily permit my child to participate in these activities and knowingly assume these inherent risks on behalf of my child.

Medical Authorization

If my child becomes ill or injured while participating in a Green Room program and I cannot be reached promptly, I authorize The Green Room Theater and Dance, its employees, contractors, volunteers, and representatives to obtain emergency medical care for my child as deemed reasonably necessary.

This authorization includes permission to contact emergency medical services (911), arrange ambulance transportation, authorize emergency medical evaluation and treatment, and obtain hospital care when necessary.

I understand that every reasonable effort will be made to contact me before medical treatment is authorized whenever practical.

I accept full financial responsibility for any medical expenses incurred.

Medication Policy

The Green Room Theater and Dance does not routinely administer medication unless specifically authorized in writing by the parent or guardian and permitted by applicable law.

If my child requires emergency medication, such as an EpiPen or inhaler, I agree to provide the medication along with written instructions for its use.

Release of Liability

To the fullest extent permitted by California law, I release and hold harmless The Green Room Theater and Dance, its owners, directors, employees, instructors, contractors, volunteers, and host facilities from any and all claims, demands, causes of action, damages, or liability arising out of my child's participation in Green Room activities, except for claims resulting from gross negligence or willful misconduct.

I understand that this release applies to all Green Room programs, including classes, rehearsals, camps, workshops, performances, and related activities.

Parent/Guardian Acknowledgment

I certify that I am the parent or legal guardian of the participant named above.

I have carefully read and understand this Medical Authorization, Assumption of Risk, and Liability Waiver. I understand that I am giving up certain legal rights on behalf of myself and my child, including the right to bring certain legal claims. I sign this agreement voluntarily and understand that it will remain in effect for all Green Room Theater and Dance activities unless revoked in writing.
