Release and Waiver of Liability
This release and waiver of liability (this "Release") is executed by "I" or "me" (Parent/Guardian name entered in form below) on behalf of my child (Child’s name entered in form below) in favor of The Green Room Theatre & Dance LLC, and its directors, employees, contractors, and volunteers (collectively, "The Green Room").
Release and Waiver: I hereby fully and forever release and discharge The Green Room from, and expressly waive, any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, that may arise from my or my child’s (or children’s) participation in classes and performances produced by or associated with The Green Room. I agree not to make or bring any such claim or demand against The Green Room, directors, employees, contractors, and volunteers, and fully and forever release and discharge The Green Room and foregoing persons from liability under such claims or demands.
I understand that this release discharges The Green Room from any liability or claim that I may have against The Green Room with respect to any bodily injury, personal injury, illness, death, property damage, or property loss that may result from the classes or performances, whether caused by negligence of any person or otherwise.
Medical Treatment: I hereby give consent and authority to The Green Room to initiate and/or obtain medical treatment on my behalf if my child is injured or requires immediate medical attention during participation in activities relating to The Green Room. In the event that my child has a severe allergic reaction, I consent to a member of The Green Room’s volunteer staff administering epinephrine if my child is temporarily unable to carry and give him or herself medicine. I understand that The Green Room does not store epinephrine on site. I understand and agree that I am solely responsible for all costs related to such medical treatment, medical transportation and/or evacuation. I hereby release, forever discharge, and hold harmless The Green Room from any claim whatsoever in connection with such treatment by The Green Room or other medical services, including in connection with administering epinephrine.
Photographic Release: I understand and agree that during classes, rehearsals, and performances, I and/or my child may be photographed and/or videotaped for internal or promotional purposes by or on behalf of The Green Room. I grant permission for The Green Room to use any photos or recordings that may include my child’s image, likeness, or voice in materials related to their activities, including promotional materials and on The Green Room websites without further permission or compensation.
